Driving directions
We are located at the Centerplace I Medical Plaza in Flower Mound, TX. From I-35 E in Lewisville, exit FM 1171 (Main St./Cross Timbers Rd.) and go west toward Flower Mound. Approximately 4.5 miles west of I-35 on FM 1171 (Main St./ Cross Timbers Rd.) you will cross FM 2499. After crossing FM 2499 there will be a Sonic Drive In on the left. Turn left after the Sonic onto Churchill Dr. and take the first right into the Centerplace I Medical Plaza parking lot.
We are on the second floor in suites 212 and 215.
